  1. No, but it's best to integrate said entrance with the design.

  2. As NevePlaysDota said, it's not the main entrance. The main entrance should be on the direct opposite side. The entrance into the cliff doesn't need to be integrated with the building at all, but it would be cool if you do. Feeler to move it around a bit if you need to.
