That is my Intel Graphics Driver Version.
Also, I tried removing the mods and placing them in one-by-one.

I found that the mod that caused the problem was Optifine! (D3 1.8.0 Ultra)
Is it possible that Optifine and Intel don't work together?

I have OptiFine and Intel. They both work.

Penguin do u have that exactt version of GPU driver?

Your driver is from 2013. Is there any way you can update it either from your device manufacturers website or Intel's site?

That's the newest version.
I guess they stopped making them for Windows 7.
I think they stopped when they started W8.

Since Optifine is the cause of the crashes, can you live without it and just use the other mods instead? Maybe use mcpatcher to do your hd texture pack.

Without Optifine I get 2 FPS... -_-

Is the two fps with the hd texture pack? Try removing it or using a super small resource pack.

It's not HD. I think its 32 or 64.

I tried Vanilla too. I still get the same FPS with and without the TP.
I am now using D2 Optifine, but I would still like to fix this problem.

Does Optifine D2 work fine? If so, you might want to stick with that for the time being.
